PTA PRESIDENT: Not Getting Your PTA Mail?
If you are not receiving mailings from National or NYS PTA, then your PTA probably has one or both of these two problems:
--It has not filed the Annual Officer Contact Form/Form A for 2008-09
OR
--It has not paid insurance for 2008-09
Both problems are easy to fix. Just follow the steps below.
To File the Annual Officer Contact Form:
Go directly to the state PTA website: www.nyspta.org. On the left side of the page you will see a link for the Form A. You will need the current president’s name, address, phone and e-mail and the same information for the unit treasurer. You will also need your Federal EIN number and your unit code number (18-XXX). Both are listed on the front page of your bylaws. Once the Form A information is received, your membership packet will be released.
To Pay PTA Insurance:
Your unit should make out a check for $255 to NYS PTA. Be sure to put your unit code number, a notation that says “2008-09 insurance fee,” and two authorized signatures on the check. Mail the check with your invoice to the state office; if you do not have an invoice for payment, attach a note explaining that the check is to pay your PTA’s insurance bill for 2008-09. Send the payment to the address below and you are done!
